Why your choice at 65 matters more than people realize

When you turn 65, you have a one-time 6-month Medigap Open Enrollment Period where insurers must sell you any Medigap plan at the standard rate, regardless of your health. After that window, in most states, Medigap insurers can use medical underwriting and may decline you or charge more for pre-existing conditions. Medicare Advantage, by contrast, has no medical questions. You can move into Advantage from Original Medicare anytime during the Annual Enrollment Period (Oct 15 to Dec 7).

The catch: if you pick Medicare Advantage at 65 and later decide you want Medigap, you may not qualify for it. This is why a licensed agent should walk through both routes with you before you decide.

Your Initial Enrollment Period (IEP) is a 7-month window: it starts 3 months before the month you turn 65, includes your birthday month, and ends 3 months after. Some people qualify earlier, typically after 24 months of receiving Social Security Disability Insurance, or with certain conditions like End-Stage Renal Disease or ALS. If you are already collecting Social Security at 65, you are usually enrolled in Parts A and B automatically.
